Background Investigation
By request of our AMEC leadership, if you perform administrative duties within the church or if your ministry affects children, please take a moment to fill out a background investigation form. Follow the link below to access the form. There is a fee of $7.50 and will collected after your information is processed, no money is required at this time. The form will take approximately 10 minutes to complete. Please use the cheat sheet to gather the needed information prior to your data entry as there is a 30 minute time limit. All information is stored in a secured database. Your information will not be shared/sold to any third party organizations. Thank you for your cooperation.
